Ayuda Line: How do I obtain a health certificate?

Answer: With summer just around the corner, those looking for summer jobs at a health-regulated establishment will need to obtain a health certificate.

If you have any doubts about whether you’ll need one, you can confirm with your employer.

According to the Guam Department of Health and Social Services, to obtain the certificate you must complete the health certificate application, which is available at the Division’s Processing Center Section, located in Hagåtña at 155 Hesler Place 8 a.m.-noon and 1-4 p.m. Monday to Friday, except on observed government of Guam holidays.

If it is your first time applying for a certificate for a food facility, you will need to show the result of your PPD, or tuberculosis, skin test taken within the last six months and attend and pass the food safety training course, which will be scheduled and provided by the processing center.

If you are renewing your certificate for a food facility, you will need to complete the Renewal of Eating and Drinking and/or Food Establishment form, also available at the processing center. Additionally, you must attend and pass the Health Certificate Food Safety Refresher Training Course.

If it is your first time applying or renewing your health certificate for cosmetology, tattoo, institutional (nursing home, adult care, child care and correctional facility) and therapeutic massage, you must complete the health certificate application and provide any required documents listed in the application.

All applications must be submitted to the processing center for review and processing. Applicants will be issued a health certificate when all completed documents are submitted and the fee is paid.
